What Actually Happened. What It Means Next Time.
Independent after-action review facilitation. Decision-tree reconstruction. Findings with named owners and deadlines, captured while details are fresh.
AARs run in-house surface symptoms, not decisions
The same team that responded runs the review. Internal politics shape what gets written. Findings come out generic. Owners are vague. Deadlines slip. By the next exercise the same gaps appear in the same places. An independent facilitator changes the tone. The question is no longer who is at fault. It is what decision needs to change before this happens again.
How We Run It
Best engaged within seven days of the event when memory is fresh and appetite to act remains. A four-step approach: hot-wash within 72 hours facilitated by an independent consultant, one-on-one structured interviews with key responders and decision makers, timeline and decision-tree reconstruction showing where the response branched and what alternatives existed, and documented findings with named owners and concrete deadlines.
How Findings Move to Action
A finding that sits in a PDF is not a finding. It is a paragraph. The portal is how findings move to closure through owner assignment with deadline and closure criterion, corrective action tracking until closure, quarterly status visibility to leadership, open findings carrying forward into the next exercise scope, and an audit trail of what was found, who owned it, and when it closed.

Industrial operators after a tabletop, functional, or full-scale exercise. Healthcare systems after a coordinated response or drill. Operations teams after an actual incident, near-miss, or process-safety event. Critical infrastructure operators after a regulatory inspection or coordinated multi-agency response.
